All clients must answer the following wellness questions upon booking an appointment:

Have you been exposed to anyone who has coronavirus? If so, when?
Have you had any of the known symptoms associated with the coronavirus?
Have you traveled by commercial airline, bus or train within the past 14 days?
Will you agree to inform the office if you are exposed or test positive for COVID-19 after your visit?
Appointment Procedure: Safety begins before we step into the office.
Please take your own temperature for 3 days prior to your appointment. Your temperature will also be taken digitally before entering.
Clients are asked to wait outside in your car and you will be called in when it’s time to enter.
Apply an alcohol-based sanitizing gel to your hands prior to entering the office and before or after touching any surfaces within the office.
A new, clean, disposable face mask is required to be worn upon entering the office and during a treatment visit. (Please wash your hands and faceprior to applying a new mask for your appointment.) Bring a 2nd new mask in a clean bag with you to your appointment as well.
So that most facial, chin, jaw, and neck areas can be treated, the mask may need to be folded in and up, on the front of the chin. Tape can then be applied at home or at your appointment, to hold the fold place. Practice beforehand. Children’s size pleated masks are 5 1/2 “ wide and also recommended for facial areas.
Wash your hands again after your mask fits securely. Do not touch your face or your mask once it is set.
Touching your cell phone is prohibited in the office prior to your treatment.
